TOURNAMENT SPONSORS & EXHIBITION GAME ORGANIZERS INSURANCE PROGRAM FOR 2007

• Provides Blanket Insurance whether players are USL members or not.
• This coverage can only be purchased by Tournament or Exhibition Game organizers to cover a Tournament or Exhibition Game event. Individual teams cannot purchase this insurance.
• Insurance rates based upon the number of teams, subject to a $200 minimum premium.
• Full payment must be received by Bollinger’s office at least 5 days prior to your event in order to issue your certificates and process the insurance.
• Please make check payable to “Bollinger Insurance.”
• Within 10 days after the end of your event, submit a roster with player’s names, full addresses and phone numbers for claim verification purposes.
  Please note: No Claims can be paid without a full roster on file.
