ONE DAY CLINIC & INSTRUCTIONAL CLASSROOM INSURANCE PROGRAM FOR 2007

• Provides Blanket Insurance coverage whether players are USL members or not.
• Program designed only for One Day Clinics or for single or multiple classroom clinics only. Outdoor clinics over one day would fall under the Camp Insurance Program.
• One day games are not covered under this program, please refer to the Tournament/ Exhibition Game Insurance program.
• Insurance rates are based upon the age group and number of participants, subject to a $100 minimum premium and US Lacrosse Administration Fee.
• Full payment must be received in Bollinger’s office at least 5 days prior to the start of your events in order for your certificate to be issued and the insurance processed.
• Please make check payable to “Bollinger Insurance.”
• Within 10 days after the end of your event, a roster with player’s names, full addresses and phone numbers must be submitted for claim verification purposes.
  Please note: No claims can be paid without a full roster on file.
